We can only accept new patients who live in our practice area. However, if you live out of the area but work in our catchment area you may be able to register for access to appointments and prescriptions. There are a few restrictions on follow up care after surgery or treatment and on the ability to make a home visit for example. Talk to us to find out more.

Register with the surgery

We use an online service called Register with a GP surgery that makes it easy to register with this us.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. If you are unable to access and complete the form independently, our reception staff will be happy to help.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register. If you are unable to complete the online form yourself, ask a friend or family menber to help you. If this is not an option please contact our reception team who will be happy to complete the form on your behalf.

The service is designed and run by the NHS. It aims to cut practices’ workloads and make GP registration easier for the public.

When you register, we will ask you to make an appointment to see either the practice nurse or healthcare assistant for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.

Once you have been accepted as a patient your medical records will be transferred to us. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

If you have any medication on repeat prescription, we recommend ordering a months supply to ensure you are not left without during the transfer process.

The registration process will take around 15 minutes to complete. You will need to complete the process in one session.

We'll ask you for, if you have it, your:

  • name and address
  • date of birth
  • previous UK GP surgery
  • health and medical information
  • contact details

You can provide a temporary address or other contact address if necessary.

If you are registering for a UK GP for the first time you may also need, if you have one:

  • European Health Insurance Card (EHIC)
  • S1 form

You DO NOT need your NHS number but it might make the process easier. You can find your NHS number by logging in to a GP online service or on any document the NHS has sent you, such as your:

  • prescriptions
  • test results
  • hospital referral letters
  • appointment letters

Your answers will help us find the correct medical records. We use these records to offer relevant and appropriate healthcare services for the person being registered.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 8am to 6pm.

Linked profiles in your NHS account

 Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
